Large volume liposuction can be done very safely . Dr Bruce Halperin and I have been writing about and teaching safety in liposuction for the past 20 years. Our most recent contribution to the literature was a chapter in the 4th edition of ANESTHESIOLOGISTS MANUAL OF SURGICAL PROCEDURES, chapter 11.2, pages 1098 forward. Prior to this, Dr Halperin and I wrote the largest large volume series of cases ever reported in the world's literature. This was a series of 631 Large Volume Liposuction cases: Larger Volume Liposuction: A Review of 631 Consecutive Cases over 12 years, reported in Plastic and Reconstructive Surgery, November, 2001, Vol. 108, No. 6, Pages 1753 onward. If you would like a copy please call my office. This article emanated from the Departments of Plastic Surgery and Anesthesiology, Stanford University Medical Center. Every case was done by Dr Commons 100%. At present Dr Halperin and I are preparing a follow up 10 year later article presenting another 381 cases done since the report in 2001. This will make a total of 1012 large volume cases I have done. This article will include our updated and current thoughts on the subject of Large Volume Liposuction. In brief, we feel Large Volume is safe in our hands and we do have strong feelings about guidelines. There are many candidates for large volume liposuction that are in the 50 pounds or less overweight group. When patients exceed the 50 pounds overweight limit, there are still some perfectly good candidates for serial reduction or reductions that focus on the most problematic areas. These include a very large stomach or a very large and disproportionate buttocks etc. There are certainly some good candidates in the 75 pound overweight group but we stress a careful work before surgery in all cases and especially as weight increases. So are there any candidates in the 100 pound and over, overweight group. Possibly? The person who is 100 pounds overweight that has a very disproportionate abdomen can be helped and inspired by a large volume liposuction focused on the abdominal area. Large Volume Liposuction is generally defined as any liposuction over 10 pounds. We believe there are many people in the 10 to 15 pound removal of fat group but after that there are very few in the 15 to 20 pound group. Beyond 15 pound fat remova,l it is safer and ideal to stage the procedures over 2 or more surgeries.
